You are invited to participate in a research study to learn about your kidney health using the Kidney Check test.

The Kidney Check test is an at-home urine test to detect early signs of kidney damage by using a test kit and a smartphone app. The kit was created by Healthy.io and they are sponsoring this study. This kit has not yet been approved to be sold in the United States by the Food and Drug Administration (FDA).

The test is easy and takes a few minutes. It is designed for people of all ages and can be done in the comfort of your home. The Kidney Check service consists of a kit and a mobile application that effectively turns a smartphone into a urine dipstick analyzer. The application guides you, performs the scan, displays the results in the app, and provides a link with general information about albuminuria and the test results. When the service is provided to you on behalf of a health plan, based on both the plan's preference and your test results, a notification will be available to you and your primary care physician so you can discuss the test results with your physician during your next appointment. This test is not intended to give you a diagnosis, so any concerns about your kidney health should be discussed with your personal primary care physician. The purpose of this study is to determine people’s adherence to perform a urine test from the comfort of their home by using the Kidney Check test. This study will also evaluate user experience with the Kidney Check service.

This study sponsor will collect the following information:

• Your name, email address, physical address, and phone number

• Information about the number of people who requested a Kidney Check test and have used it

• Test kit results from the smartphone app

• Where the service is provided to you on behalf of a health plan - we will also collect additional information provided to us by the health plan All the personal information collected in this study will be used solely to provide you with the Kidney Check service.

It is expected that at least twenty thousand men and women age 18 and older will participate in this study. Participation in this study will last about 10 minutes or the time it takes you to perform the Kidney Check test. There are no risks or discomforts expected from completing the Kidney Check test. The only possible risks from participating in the study is the potential for loss of confidentiality if your personal information and test results are seen by someone not involved in the study. The steps to protect your confidentiality are described below.

Participating in this study may give you more information about your kidney health, but this cannot be guaranteed. The information learned from this study may help patients with kidney disease in the future.

Costs/Compensation

There is no cost to you for participating in this study and using the Kidney Check service. You will not be paid for your participation in this study.

Financial Disclosure

The study doctor is an employee of the study sponsor of this study. His employment is not dependent on the study results, and there is no incentive for specific results. If you have any concerns about this financial relationship, please ask the research staff.

Voluntary Participation

You do not have to participate in this study, and you can end your participation at any time without any penalty. If you do not use your Kidney Check test your contact information will be deleted and you will be removed from the study.

Confidentiality

Contact information, such as your name, e-mail, phone number and mailing address will be used solely to provide you with the Kidney Check service. Personal information will be stored in an encrypted password-protected secured electronic folder. Only people with secure access will see your personal information. Your Kidney Check test results will be given a study number and your personal information will be kept separate from your results. Once the study is finished, your contact information will be deleted. Any papers, articles or presentations about this study will not include any information that could identify you. At the end of study, if decided to complete the voluntary survey, you may be asked if we may contact you to get your feedback on the service, only if you answer “yes”, will we use your personal information to contact you. By verifying your phone number on the Kidney Check app, you confirm that you are 18 years old or above, allow sharing of personal health information with your health plan and primary care physician, voluntarily agree to participate in the research study, accept Healthy.io's terms of use and privacy policy, and understand the Kidney Check device is intended for investigational use purposes only. At least one text may be sent to you with instructions to download the Kidney Check app.
